COMMUNITY SUPPORT SERVICES
Each person’s needs are different. The Allen County Board of Mental
Retardation and Developmental Disabilities offers services based on individual
needs and desires. Eligible individuals receive assistance in developing a
service plan that is "person-centered" and tailored to suit a person’s
specific needs.
Community Support Services provides Case Management Services,
Supported Living Services, Residential Services, and Family Resource
Services tailored to the individual's needs.

SUPPORTED LIVING: Residential supports and services are offered which enable
individuals to live in the settings of their choice within the community.
Services generally include assistance with shopping, banking, budgeting, meal
preparation, leisure activities, and general supervision.

RESIDENTIAL GROUP HOMES: Services are coordinated for individuals living in
group homes in Allen County. The group homes are operated by private providers
who contract with the ACBMR/DD. Residential Services is responsible for service
planning, monitoring of services, and quality assurance.

FAMILY RESOURCE SERVICES: Assistance is offered to families who have a
family member in their home who has mental retardation and/or developmental
disabilities. Services and supports may include respite care, adaptive
equipment, home modifications, etc.

HOUSE SERVICES: The Allen County Board of MR/DD works cooperatively with LODDI, Inc., a non-profit housing corporation that provides affordable housing
to persons with disabilities in Allen County.

| |
CASE MANAGEMENT SERVICES: Case Managers assist enrollees in the development
of person centered plans and coordinate and monitor all services received by
eligible individuals. |
