How will Help Me Grow help my
family?
-
Provide information about
your child's shots, diet, and growth during home visits
-
Bring together special
services such as prenatal visits, newborn visits, home visits and other
help for your family
-
Provide checks for all
areas of growth, especially if there is a suspected delay
-
If you need ongoing
services, a service coordinator will help your family develop a plan. This
plan is called the Individualized Family Service Plan (IFSP). The IFSP is
a roadmap to help meet your
family's needs and goals.
What will happen if I call
Help Me Grow?
The person answering the
phone will need to know your name, address, phone number, your child's name
and birth date. They may ask what your concerns or needs for your child are.
An appointment will be
scheduled to meet with a staff member.
Whether your family has
medical, educational, social, emotional or financial needs, Help Me Grow
staff will help you find and get services.
Services are confidential
and available to families at no cost, no matter what your income.
